Information you choose to provide

Contact forms collect the fields shown, such as your message, email, organization and permission to reply. Some fields are optional. Contribution inquiries collect a proposed amount and frequency with contact details; the form does not collect card or bank information.

Email subscriptions collect your email, optional first name, chosen interests and consent. A confirmation link verifies the address before updates are activated.

Keep private child records, health histories, identity documents and financial access credentials out of ordinary inquiries. These forms are not clinical records or a case-management service.

How the information is used

Inquiry information is used to receive, route and respond to the request and maintain a record of related correspondence. A reference identifies a submission. Subscription information is used to confirm the request, manage interests and send the updates selected.

Permission to reply about an inquiry is separate from choosing email updates. Leaving an updates choice unchecked does not prevent an inquiry from being submitted.

Website and delivery providers

Cloudflare provides website hosting, security, database storage and email delivery services used by this website. Inquiries are stored and notifications are sent to NPF's designated inbox. Service providers process information for these website and delivery functions.

Requests include technical information needed to operate and protect the service. Form rate limiting uses a derived identifier and a limited time window. We do not use inquiry messages to create advertising profiles or sell them as marketing lists.

Confirmation links expire after 48 hours. Unconfirmed subscriptions and request records are removed by the scheduled housekeeping process after 30 days as applicable. Confirmed subscriptions remain until changed or removed. Inquiry records and necessary correspondence are retained to handle and document the request; ask about a specific record if you want it reviewed or deleted.

Inquiry notifications are delivered to the designated NPF inbox. Contact NPF about the inquiry reference if you need to ask about your record.

Mom’s Pick Awards and product-registry concerns

The NPF Healthy Products Registry publishes approved product identity, recognition scope, award year, references, dates, status and record history from Mom’s Pick Awards. Private application evidence, applicant contact details and physical-order information are not published in the registry.

The registry concern form collects your name, reply email, the relevant award reference and the concern you describe. The request is stored in the Mom’s Pick Awards service operated by NPF so authorized program staff can check the record and reply. Cloudflare provides its database, file and email infrastructure. Submitting a concern does not subscribe you to email updates.

Concern records are retained for 24 months after closure, subject to an unresolved matter or an applicable obligation. Private access, correction and deletion requests are verified before information is disclosed or changed. Public product history remains available to identify current and historical recognition.

Access, correction and deletion requests

Choose Privacy on the contact form. Explain the record or request and provide enough information to locate it. NPF may need to verify the request before disclosing or changing private information. Some records may need to be retained for an applicable obligation or an unresolved matter.
