Information to include

Provide the public reference or record URL, describe the specific difference or issue, and give the date and source where you found it. An official product page, public notice or other verifiable link is useful.

Do not provide a child's name, photograph, diagnosis or unrelated private family information. We need facts about the product or award statement, not sensitive personal details.

Concern form

Send a record concern

Identify the product, dates and specific information. Do not include personal information about children.

What happens next

The reported facts are checked against the record and relevant source information. Clarification may be requested from the reporter or the authorized recipient. If a correction or status change is warranted, the public record is updated through the authorized process with an appropriate dated note.

Private reporting details and unverified allegations are not posted to the product record.

Urgent product matters

Use the manufacturer's contact route and the appropriate official authority for an urgent product issue. The registry concern form is not an emergency service and does not replace a product recall or incident report.

For relevant U.S. consumer-product information, consult CPSC recalls and safety warnings. CPSC directs consumer product incident reports through SaferProducts.gov. For products within FDA's remit, consult FDA recalls and safety alerts.

Recipients and applicants

An authorized recipient seeking to correct its own record should use the Mom’s Pick product-change service, which verifies recipient authority. An applicant questioning its private decision should use decision review rather than a public concern report.

Your report and reply details

A saved report receives a service reference. Use that reference and the same reply email if you follow up. A save error means the report was not received; correct the field or retry.

Reply details and the report are used to assess and respond to the concern. The approved public record is kept separate from the reporter's private information.
